How to Write a Simple Power of Attorney
Most states have laws on the books governing how a person (the "principal") can delegate the authority to conduct his or her legal affairs onto another person (the "agent"). This type of arrangement is called a "power of attorney." The Power of Attorney document authorizes a person to make decisions for you when you are unavailable or unable to do so yourself. Most states have pre-made forms you can use to write your own power of attorney.

Find your state's Power of Attorney Act. Call your State's Bar Association, contact a local court or use the Internet to search for your State's Power of Attorney Act. Fill out the form included in the statute.
2In the form, identify yourself by giving your full name and address and explain that you are designating certain powers to your agent. List your agent's full name and address.
3Describe the powers you want your agent to have. For a general list of powers, see the link in the Resources section below. Tailor the powers to your specific needs or desires. Do not include powers that you do not want your agent to have.
4Take the document to a notary public. Sign and date the document in the notary's presence and have the document notarized.
